1. CONSISTENCY IN FIRM NAME
1.1 The firm name on all required documents must be the same, as this is the name in which the firm will be certified.
1.2 If using a fictitious name, please provide authorization documentation from the State of Florida.
2. STANDARD FORM 330 (SF 330)
2.1 The firm must submit a current SF 330 (Part II – General Qualifications) to indicate the firm’s experience. A SF 330 (Part II) indicating that it was either prepared and/or signed prior to December 1, 2008 will not be considered current for certification purposes.
2.2 Firms anticipating having services performed from offices outside of Hillsborough County shall also include an additional SF 330 (Part II) for each of the offices to indicate that office's experience in providing services.

3. REQUEST FOR TAXPAYER IDENTIFICATION NUMBER AND CERTIFICATION – FORM SUBSTITUTE W9
3.1 The firm must submit

4 .DEPARTMENT OF BUSINESS AND PROFESSIONAL REGULATIONS (DBPR) REGISTRATION
4.1 The firm must submit evidence of registration with DBPR to practice its profession in State of Florida.
4.2 The current DBPR license is the preferred form of documentation.
4.3 Licenses may be submitted for certification in any, or all of the following disciplines:
4.3.1 Architecture
4.3.2 Engineering
4.3.3 Landscape Architecture
4.3.4 Surveying and Mapping
4.4 The area(s) of certification will be determined by the license(s) submitted.
5. CORPORATE CERTIFICATE OF STATUS
5.1 If the firm is a corporation, it must provide proof that is properly chartered with the Florida Department of State.
5.2 A current Corporate Certificate of Status from the Florida Department of State is the preferred form of documentation.
5.3 This certificate must show the firm in active status, at least, through 12-31-08.
